## How to Fill Out Form DS-11

### What Is Form DS-11 Used For?

Form DS-11 is used to apply for a new U.S. passport if you’re an adult who wants to receive your first passport or a minor under the age of 16. You can also submit it for passport renewal in the following circumstances:



- Your previous passport was issued when you were younger than 16
- Your previous passport was issued more than 15 years ago
- Your previous passport was lost, stolen, or damaged

### Who Should Use Form DS-11?

Anyone who wants to apply for or renew their U.S. passport and who fits the criteria should use Form DS-11. If you don’t match the criteria, you may be eligible to use Forms DS-82 or DS-5504.

### Are There Any Form DS-11 Fees?

There are two types of Form DS-11 fees: adult and minor fees. If you apply for a first-time passport or a renewal as an adult, you’ll have to pay $35 for facility acceptance and $160 for a passport book and card (you can also get only one of these at $130 for a book and $30 for a card).



If you’re submitting Form DS-11 for a minor, the fees are somewhat lower: $115 for the book and the card, while separately they cost $100 and $15, respectively. The facility acceptance fees remain the same.

### How and Where Do I File Form DS-11?

Form DS-11 must be filed in person at an acceptance facility, a passport agency, or a U.S. consulate or embassy if you’re abroad. The appointment should be scheduled in advance in person or by calling the National Passport Information Center (NPIC). Alternatively, you can visit the U.S. Department of State and schedule it there.

### How to Check Form DS-11 Status?

You can track your application status online at Passportstatus.state.gov or call the NPIC if you have any further inquiries. However, don’t expect any changes in under two weeks.
