Approval also includes an irrevocable waiver of Social Security and Medicare benefits during the exemption period.

### Who Should Use Form 4029?

You may use Form 4029 if you are a member of and follow the teachings of a recognized religious group that opposes insurance benefits, including Social Security and Medicare. The religious group must meet specific IRS requirements, including continuous existence since December 31st, 1950. This date aligns with the original Social Security Act provisions establishing this exemption; groups formed after this date do not qualify.

### When Is Form 4029 Due?

Form 4029 must be filed by the due date of your federal tax return (including extensions) for the first taxable year in which you have self-employment income and meet the religious group membership requirement. For most filers, that means April 15th of the year following your first year of qualifying self-employment income (or October 15 if you file an extension).

Once approved, the exemption is permanent, i.e. there is no annual refiling required. However, the exemption may be voided if the individual leaves the qualifying religious group or the group no longer meets IRS requirements.
